This form is to gain permission to use photos and/or video of you and the child/infant in your care that have attended a Bond massage or yoga session.
 

The photos will be used for some, or all of:

-The Bond website
-Bond social media (Facebook and/or instagram)
-Video pieces that will appear on the above, and possibly YouTube
-Press releases for local media

-Print materials such as leafets or adverts.

We won't identify you or your child by name, unless we have received prior permission
